Why case-note selection matters so much
A common mistake is treating the notes as a list of information that has to be transferred into the letter.
The reader does not need everything. What matters depends on who you are writing to, why you are writing and what that person needs to know next.
This is why writing feedback needs to go beyond correcting grammar. It should show you where your selection, organisation or purpose became unclear.

OET Speaking without memorising a script
Memorised phrases can be useful in moderation, but they become a problem when you are waiting for an opportunity to use them instead of listening to the patient.
Effective practice develops the ability to handle common communication functions naturally: explaining, reassuring, asking questions, checking understanding and discussing next steps.
Role-play practice can include:

Writing everything
Including too many case notes can make an otherwise accurate letter difficult to follow.

When the reason for the letter is not immediately clear, the rest of the information becomes harder for the reader to interpret.
Speaking from a script
Memorised responses can make role-plays sound mechanical and make it harder to react to what the patient actually says.
Weak timing

Studying everything equally
