Why OET Writing can be difficult even for experienced doctors
Doctors are accustomed to processing large amounts of clinical information. In OET Writing, however, including more information is not necessarily better.
You have to understand the purpose of the letter, identify what the reader needs to know and leave out details that do not help that purpose.
This is where many otherwise strong candidates become too detailed, repeat information or organise the letter around the case notes rather than around the needs of the reader.
Writing practice should therefore involve more than producing letter after letter. Correction needs to show you what to change and why.
OET Speaking is a clinical conversation, not a memorised speech
Doctors sometimes prepare a collection of phrases and then try to insert them into every role-play. That can make the interaction sound rigid and distract from what the patient is actually saying.
A better approach is to become comfortable with the communication functions that repeatedly appear in consultations: explaining, reassuring, checking understanding, exploring concerns and discussing next steps.

- clear explanations;
- empathy without sounding scripted;
- signposting the conversation;
- responding when the patient disagrees;

Why a doctor with good English can still struggle with OET
Too much information
Clinical knowledge can make it tempting to include every detail, particularly in Writing. OET requires selection as well as accuracy.
Overly technical language
The language that works between clinicians is not always the language a patient needs during a Speaking role-play.
Weak time control
Understanding the task is not enough if too much time is spent on one question or one part of the test.
Memorised responses
Prepared expressions can help, but relying on a script makes it harder to respond naturally when the conversation changes.
Studying every skill equally
Your weakest area may need considerably more attention than a sub-test in which you are already performing consistently.
Practising without feedback
Repeating the same task is less useful when the same structural or communication problems are repeated each time.
